Visceral fat on a DEXA scan: what the VAT number means

Of every number on a DEXA report, the visceral fat estimate — usually labeled VAT — is the one with the most health signal and the least explanation. Here's what it measures, where the thresholds people quote actually come from, and what the number can and can't tell you.

What VAT is

Visceral adipose tissue is fat stored inside the abdominal cavity, packed around the liver, intestines, and other organs — distinct from the subcutaneous fat under your skin. The distinction matters because the visceral depot is metabolically active and is the fat pattern most consistently associated with cardiometabolic risk in the research literature. Total body-fat percentage doesn't capture it: two people at 25% body fat can carry very different visceral loads, which is why the report breaks it out.

How DEXA gets the number

A DEXA scanner can't see depth directly — it images in projection. Modern software (GE Lunar's CoreScan, Hologic's visceral-fat module) estimates the visceral component in the android region by modeling the subcutaneous layer from the body outline and subtracting it. That sounds indirect, and it is — which is why it needed validation: the key study compared DEXA VAT against CT (the reference method) in 124 adults spanning BMI 18.5-40 and found agreement of r² ≈ 0.95, concluding DEXA measures abdominal VAT precisely in both sexes.

Two honest footnotes most sites skip:

  1. The 100 cm² threshold was derived on CT, then applied to DEXA because of the validated calibration above. That's reasonable — it's what the field does — but it's an applied threshold, not one derived on DEXA itself.
  2. The 160 cm² "high" line is a reporting convention. It aggregates from CT epidemiology and appears across vendor reports, but you won't find one seminal paper that owns it the way the Japan criteria own 100 cm². We label it accordingly in the interpreter.

Using the number well

  • Match units before comparing. The thresholds above are areas (cm²). If your report gives grams or cm³, find the area line — the line-by-line guide shows where each vendor prints it.
  • Trend beats snapshot. VAT responds well to the boring levers — the evidence-based rundown covers what actually moves it — and a falling trend across scans is a more trustworthy signal than any single reading.
  • A high number is a conversation starter, not a verdict. These are population research thresholds. If your VAT sits in the elevated range, that's information worth bringing to a clinician alongside the rest of your health picture — it isn't a diagnosis, and this site won't pretend otherwise.
