How to lower visceral fat: what the evidence actually supports

Visceral fat is the fat stored deep in the abdominal cavity, packed around the organs — distinct from the subcutaneous fat that sits under the skin, and the depot researchers measure with CT, MRI, or a DEXA estimate rather than a mirror. This page is about the part everyone actually asks: what moves it. The two levers with real meta-analysis support are a sustained calorie deficit and regular aerobic exercise. Everything else is either secondary, associational, or marketing.
No promises here — the studies report averages across groups, and your result will be your own. But averages are exactly what you want when deciding where to spend effort.
Lever 1: lose weight — visceral fat goes first
The most useful finding in the literature: when people lose weight, visceral fat tends to leave disproportionately early. Chaston and Dixon's systematic review — 61 studies, 98 cohort time points — found that modest weight loss produced preferential loss of visceral over subcutaneous fat, with the effect fading as total weight loss grew. Of all the factors the review examined — degree of caloric restriction, exercise, starting BMI, sex, follow-up time — percentage of weight lost was the only one that influenced the visceral-to-subcutaneous ratio.
Translation: you don't need a special "visceral fat diet." A moderate, sustained deficit is the mechanism, and the early weeks are disproportionately productive for this particular depot — on average.
Lever 2: aerobic exercise, specifically
Ismail et al.'s meta-analysis pooled 35 exercise trials in overweight and obese adults, all using CT or MRI to measure visceral fat directly:

The authors' conclusion: aerobic exercise is the centerpiece of an exercise program aimed at visceral fat. That doesn't make lifting useless — it means that for this specific outcome, the meta-analytic evidence backs the cardio sessions, not the squat rack. Do both if you like; just don't count the lifting as your visceral-fat strategy.
Secondary levers: sleep and alcohol
Sleep. A small but unusually rigorous inpatient trial (Covassin et al. 2022, 12 healthy adults in a randomized crossover design) restricted sleep to a 4-hour window for 14 days versus a 9-hour control. Participants ate about 308 more calories per day, and abdominal visceral fat rose roughly 11% — and kept climbing during the 3-day recovery period even after sleep normalized. Twelve people is twelve people, but the controlled design makes it hard to dismiss: short sleep pushed both intake and visceral storage in the wrong direction.
Alcohol. The evidence here is associational, so state it that way: in a CT study of 951 healthy Korean men, alcohol intake was positively associated with visceral fat (and negatively with subcutaneous fat), and a meta-analysis of 127 observational studies found heavy drinking associated with higher odds of abdominal obesity compared with non- or light drinking. Association is not proof of mechanism — but if you're pulling levers anyway, this one points in a consistent direction.
What does not work
Spot reduction. A meta-analysis of 13 studies with 1,158 participants (Ramirez-Campillo et al. 2022) found localized training had no effect on local fat: pooled effect size −0.03, confidence interval straddling zero, across every population and program type tested. Crunches train the muscle under the fat; they do not burn the fat on top of — or behind — it.

How to measure change without fooling yourself
Two honest instruments:
- DEXA's visceral-fat estimate — useful as a trend across scans on the same machine, but respect the noise: published same-day precision error for the VAT number is about 16%, far looser than DEXA's whole-body numbers. A single scan-to-scan drop is not a result; three scans moving the same direction are. (How often to scan covers spacing; prep consistently so the comparison holds.)
- Waist circumference — the free proxy. In an autopsy series of 201 cases, waist circumference correlated with directly weighed visceral fat at r = 0.82, and a classic imaging study rated waist among the best simple anthropometric indexes of visceral fat accumulation. Same landmark, same time of day, monthly.
A DEXA scan with a visceral-fat estimate is the cleanest baseline — and usually costs less than people expect.
